1. The no-show that ruins your Tuesday

The fix: A three-touch confirmation sequence. Text at 48 hours, text at 24 hours, text at 2 hours. Each one is specific: "Hi Sarah — just confirming your cleaning with Dr. Martinez tomorrow at 2pm. Reply C to confirm or R to reschedule."
The key is the reschedule option. Most patients who no-show were going to cancel but felt weird about it, so they just didn't show up. Give them an easy out and they'll reschedule instead of ghosting. You lose the slot for tomorrow, but you keep the patient and fill the slot from your waitlist.

2. The reactivation calls nobody makes
Every dental practice has the same list. Patients who came in 8 months ago, are due for a cleaning, and haven't booked. The list has 200 names on it. Your front desk has called 11 of them. It's been that way since March.
Nobody's doing anything wrong. There's just not enough time in the day to call 200 people when 30 patients are walking through the door.
The fix: An automated text campaign to overdue patients. Not a marketing blast — a personal check-in: "Hi David, it's been about 8 months since your last visit with us. We've got some openings next week if you'd like to get your cleaning on the calendar. Reply YES and we'll find a time that works."
Send 20 of these per day. 3-5 will book. That's 15-25 reactivated patients per week from a text your front desk person didn't have to type.
Over a quarter, that's 200+ recovered appointments. At $200 average, that's $40,000 in production that was sitting on a list gathering dust.

3. The after-hours new patient inquiry
New patient calls are worth $800-1,200 in first-year production (initial exam, cleaning, treatment plan, follow-ups). They tend to call during lunch or after 5pm — exactly when your phones go to voicemail.
A new patient who hits voicemail calls the next dentist. There is no loyalty yet. They're shopping.
The fix: An AI phone system that picks up after hours, captures the caller's name, insurance, and preferred appointment time, and texts the summary to your office manager. She calls back first thing in the morning with a confirmed slot.
One recovered new patient per week = $800-1,200/week in first-year production. The system costs less than one patient per month.
What's not worth automating
- Treatment planning. Clinical decisions stay with the dentist. Period.
- Insurance verification. Too many edge cases and payer-specific quirks. Some tools try. Most create more work than they save.
- The hygienist-patient conversation. "You need to floss more" lands differently from a human than a text message.
- Complex scheduling. Multi-appointment treatment plans with provider preferences need a human brain. Automate the simple bookings, not the complex ones.
